Alignment opens care center in East Los Angeles
Alignment Healthcare, a senior-focused health innovative model of care, is increasing access to vital medical care in the East Los Angeles area, especially for patients with disabilities and older adults with chronic health conditions, with its new comprehensive care facility, the Alignment Healthcare Center owned and operated by Citizens Choice Health Plan.
A grand opening for the new care center, located at 6201 Whittier Blvd. in East Los Angeles, will be held on Saturday, April 18, beginning at 10 a.m. The ceremony is free to all and will offer refreshments and guided tours of the new center.
The facility has already begun to see patients and it’s the first of several expansions planned by Alignment Healthcare within Los Angeles County to accommodate patients who qualify for Medicare Advantage programs with its Citizens Choice Health Plan. Through Citizens Choice, Alignment Healthcare currently provides health care access to thousands of Medicare beneficiaries in seven California counties.
The new Alignment Healthcare Center will soon feature innovative wireless technology to proactively monitor high risk patients’ health status from their home through the management of automatically triggered alerts. In the coming months, wirelessly-enabled Samsung tablets and Bluetooth-enabled peripheral health devices will notify staff at the care center of changes in the health status of the patient. The staff will then contact the patient to take appropriate steps to keep them well and out of the hospital.

As part of its growing footprint to improve access to these patients, Citizens Choice Health Plan provides its members with greater access to its network of medical offices and urgent care centers. Citizens Choice Health Plan focuses on providing access to medical and preventive care, improving patient outcomes, increasing quality of care and reducing costs through end-to-end continuous care programs such as clinical care coordination.
